Advanced Diagnostics Healthcare, a growing social services agency, has an opportunity for an RN Care Manager.
The RN Care Manager will work with the development of a Home Healthcare Program tailoring services to the elderly in homes. The RN Care Manager will also provide temporary support (i.e. medication monitoring/set-up and program plan support) to our 24 hr Customized Services and one SLS site approximately 10-12 hrs/wk.
Experience in supervision and/or providing services to elderly, project management, supervision of staff and direct services, MOSES and DICUS, and psychotropic medications are highly preferred. Knowledge of direct insurance billing and LTC insurance is also highly preferred.
Flexibility is required as the RN Care Manager Position will evolve as the Home Healthcare Program becomes established and future programs are developed.
